CCM is care coordination services done outside of the regular office visit for patients with two or more chronic conditions expected to last at least 12 months or until the death of the patient.
Chronic care management services are a specific Medicare Part B benefit. They’re intended to include all of the not-in-person work behind the medical decisions in complex chronic cases.
The overall goal of CCM is to promote your health while reducing costs.
