Chronic Care Management Marketing
Chronic Care Management Marketing - Direct to Consumer
CCM is care coordination services done outside of the regular office visit for patients with two or more chronic conditions expected to last at least 12 months or until the death of the patient.
Chronic care management services are a specific Medicare Part B benefit. They’re intended to include all of the not-in-person work behind the medical decisions in complex chronic cases.
The overall goal of CCM is to promote your health while reducing costs.
Chronic care management (CCM) is a critical component of primary care that contributes to better outcomes and higher satisfaction for patients.
Chronic care management is one of the best ways to ensure that patients who are battling chronic conditions have a better quality of life. Studies show that CCM services decrease depression, increase social activity, and promote patient engagement for these individuals.
Chronic diseases include:
- ALS (Lou Gehrig's Disease)
- Alzheimer's Disease and other Dementias
- Chronic Obstructive Pulmonary Disease (COPD)
- Crohn's Disease
- Cystic Fibrosis
- Heart Disease
